A New Community-Focused Health Service for Tackling Inequalities and Promoting Healthy Behaviours will launch on 1st April 2023.
Torbay Council has recently commissioned ABL Health to launch a new community-focused health service in Torbay called ‘Your Health Torbay’. This new service will tackle health inequalities and support the Torbay health and care system by assisting Torbay residents in making sustainable healthy behaviour changes.
ABL Health is a CQC-approved company with a passion for improving health and wellbeing in the heart of the communities that need their support the most. Established in 2009 by a GP and a teacher from Bradford, the founding members were acutely aware of the health deprivation gap between the most affluent and most deprived communities. They shared a vision to bring solution focused programmes to improve the health and wellbeing of the communities that needed them the most.
Torbay has a population of 136,218 people, with an average age of 49. The area has a higher 65+ population than the England average, and inequalities in Torbay continue to widen, with 27% of adults and 30% of children living within the 20% most deprived areas in England. Your Health Torbay aims to address health inequalities and provide support to the most vulnerable members of the community.
Your Health Torbay will offer a range of services to support Torbay residents, including specialist support for quitting smoking, Tier 2 Adult Weight Management Support, and health coaching for adults. Additionally, the service will provide web-based behaviour change information and advice, public awareness campaigns and events, and training in ‘Making Every Contact Count’ and ‘Connect Five’.
The new service will work closely with the NHS, public health, and the wider VCSE, health and care provider network to provide a seamless offer to Torbay residents. Your Health Torbay will focus on person-centred care, ensuring that each individual receives tailored support to achieve their health and wellbeing goals.
Your Health Torbay is committed to improving the health and wellbeing of Torbay residents, tackling health inequalities, and supporting the implementation of the NHS Long Term Plan.
With a focus on healthy behaviours, the service will provide Torbay residents with the support they need to achieve their health and wellbeing goals.
