Your Health Torbay – NHS Health Check Referrals
This referral pathway is for patients who have recently taken part in an NHS Health Check and may benefit from additional lifestyle support. Your Health Torbay provides free, confidential, one-to-one and group-based support to help people make positive changes to their health and wellbeing.
Referral criteria:
- Stop Smoking Support – current smokers aged 16+ wishing to quit.
- Adult Weight Management – those aged 18+ BMI ≥25 (no upper limit).
- Health Coaching – patient-led goal setting and signposting support for patients aged 18+ requiring more light touch support, for example, access to exercise of referral via our FitBay scheme .
Referred patients will be contacted by Your Health Torbay within 2 working days to arrange an initial assessment.
